A CROSS SECTIONAL PROSPECTIVE STUDY OF APPENDICITIS WITH ALVARADO SCORE IN DIAGNOSIS OF ACUTE APPENDICITIS

Abstract
Right lower quadrant abdominal pain is a common surgical diagnosis which is usually more of a
clinical diagnosis. Since this surgical condition is diagnosed on the spot, the time-tested Alvarado
score is applied. The most often applied clinical prediction rule is Alvarado scoring. In order to
compare the Alvarado score with the AIR (ACUTE INFLAMMATORY RESPONSE SCORE) score in acute
appendicitis, this was conducted. One hundred individuals over the age of 18 who had open
appendicectomy due to a clinical suspicion of appendicitis were included in this cross-sectional
observational study. In all of the specimens that have been removed, a histopathological analysis
supported the appendicitis diagnosis. The area under the curve was employed to compute the
Alvarado specificity. For both scores, sensitivity, specificity, PPV, and NPV were computed at various
cut-off values to define diagnostic zones. The study showed that patients who scored >8 were able
to undergo appendicectomy without delaying imaging and patients who scored less than 4 were
exempt from the diagnosis of appendicitis. The AIR score was 0.8 and was proven to be more
specific, in other words significant results compared to that of area under the curve of 0.75 of the
Alvarado score (p0.05).
