Assessment Of Prevalence, Risk Factors, Severity Of Insomnia & Related Quality Of Life In Anantapur District Of Andhra Pradesh-A Cross Sectional Survey

Abstract
Background: Insomnia is one of the most common issues faced by the physicians in their day-to-day practice,
though present as a standalone rare issue or being accompanied by other psychological or physiological problem.
Incidence of the problem is on a hike due increase in stressful lifestyle.
Objective: The objective was to determine the prevalence of Insomnia & the factors leading to the causation of
insomnia also to classify the prevalent cases of insomnia on the basis of severity & to determine the Quality of life
in the prevalent insomnia case & to determine the Economic burden of the disease on the semi-rural population of
Anantapur district of the state Andhra Pradesh.
Materials & Methods: The study was carried out at Anantapur district nearby villages by random selection of the
households. Investigator-reported via face-to-face interview of the sleep questionnaires Athens Insomnia scale
(AIS), Insomnia severity index (ISI), Quality of life satisfaction & enjoyment short form (QLESQSF), face validated
Risk factor questionnaire was administered to all adults (18-65years) with or without co-morbidity. Persons with
major psychiatric illness, CNS stimulant drug abusers, sleep disorders other than insomnia were excluded from the
survey.
Results: AIS identified 61.89% prevalence of Insomnia. It was high in age group of 18-28years (34.6%) compared
to other age groups (p<0.001) & wasn’t showing increasing trends of insomnia with increasing age. Insomnia is
significant in married (64.7%) compared to unmarried & widow (p<0.05). Insomnia was slightly higher in female
(56.7%) compared to the males (P<0.05). The severity of moderate clinical insomnia (96.6%) & Severe clinical
insomnia (100%) was more in the prevalent group compared to non-prevalent group (p<0.001). More than 50%
of subjects are enjoying a quality life despite being insomnia prevalent from the Q-LES-Q-SF questionnaire
(p<0.001). Risk factors with adjusted odds ratio found sleeplessness due to family pressure[adjusted odds ratio
(aOR) = 3.539, p<0.001]; co-morbidity [persistent pain (aOR) = 2.499, p<0.05] [CVS disorders (aOR) = 3.389,
p<0.001]; [altered sleep wake schedule in following routine (aOR) = 4.269, p<0.001; fear, guilt, hunger (aOR) =
5.286, p<0.05; Others in following routine (aOR) = 4.464, p<0.001] [anxiety, depression, others in the mental
health condition (aOR) = 10.58,4.24,26.46, p<0.001] [phobia of sleeping alone, alone in dark, in dark (aOR)
=2.778,4.752,8.042, p<0.05] [mental health status of poor & fair (aOR) = 23.00, 16.38; p<0.001].
Conclusion: The prevalence of insomnia was high in the semi-rural areas of Anantapur district of Andhra Pradesh
mainly due to modifiable, treatable risk factors with moderate to severe clinical insomnia among the prevalent
subjects, although the Quality of life, enjoyment& satisfaction was maintained among more than half of the
subjected population still there is a need for the improvement in knowledge & adequate validated screening
methods for insomnia for promoting even better quality of life.
