BEHIND STANDARD WARD FOR IN-PATIENT HOSPITAL THE INDONESIA’S POLICY ON NATIONAL HEALTH INSURANVE

Abstract
Indonesia has implemented a national health insurance (NHI), known as Jaminan Kesehatan Nasional (JKN), since January 2014. Meanwhile, efforts to implement the national social security system have been carried out since 2004 with the issuance of Law Number 40 Year 2004 regarding the National Social Security System (SJSN Law). JKN Program membership reach 86.8% of the total population or equal to a quarter billion people. This achievement becomes the largest national health insurance coverage that has ever been achieved within less than a decade. However, JKN members have not received equal services in terms of hospitalization. The service is still vary due to the class division based on the amount of premium paid by each member. This is contrary to the mandate in the SJSN Law that hospital inpatient services in the JKN program need to be carried out in the form of ‘standard class’. This research aims to build a policy on standard class which in 2019 when this research was conducted there was no concept or regulation on it.
This research is policy research with a qualitative research design. Several approaches are used in the research: Systematic Literature Review (SLR), policy review, Focus Group Discussion, in-depth interviews, and experts panel. The results show that some countries apply standard ward for inpatient hospital services, but there is also a country that applies tiered classes like Indonesia. This study proposes several standard ward options, namely the JKN PBI (premium assistance beneficiaries) and Non-PBI Standard ward and the JKN Standard ward. Its implementation needs to consider equity (equal rights), existing conditions of hospital inpatient care (availability of beds), potential ability to pay contributions, as well as government fiscal space for the allocation of PBI contributions.
