Comparison of the Efficacy and Safety of Transverse Parallel Compression Suture with Conventional Surgery in Pernicious Placenta Previa Treatment

Abstract
Background: Pernicious placenta previa is still challenging in the field of Obstetrics. In earlier years, different surgical techniques have been put forward to manage hemorrhaging related to placenta previa.
Purpose: To compare the safety and efficacy of transverse parallel compression suture used for haemostasis lower uterine segment bleeding of pernicious placenta previa in comparison to conventional methods.
Methods: The study population consisted of two groups: Group (1) (n=37) underwent transverse parallel compression suture approach and Group (2) (n=43) underwent conventional approach. A transverse parallel compression suture was instantly carried out to control LUS hemorrhage in Group (1). In the absence of the transverse parallel compression suture, conventional approaches such as figure-eight suture at the hemorrhage point, ligation of uterine arteries, gauze or balloon tamponade were employed.
Results: The mean blood loss in Group (1) was (1772.3 ± 85.14) and (1822.31 ± 77.46) ml in Group (2) (p=0.709). The mean operation time in Group (1) was (94.78 ± 9.18) and (129.56 ± 9.98) minutes in Group (2) with highly statistically significant variance between both groups (p<0.001). The mean ICU stay in Group (1) was (2.97 ± 0.72) and (4.12 ± 0.76) days in Group (2) with highly statistically significant variance among both groups (p<0.001). The mean follow-up in Group (1) was (30.55 ± 13.86) and (38.45 ± 18.60) months in Group (2) with statistically significant variance among both groups (p=0.034). There was none statistically significant variance among both groups as regards extra hemostatic %, post operative stay and complications (p>0.05).
Conclusions: Transverse parallel compression suture is a simple, efficient, and safe technique for stopping hemorrhaging from the LUS in females with pernicious placenta previa.
