Effectiveness of a primary care intervention to improve health goals in the elderly

Abstract
Introduction: Healthcare systems worldwide are reorganizing primary care to be proactive rather than reactive. Objective: To determine the effectiveness of the EPFC intervention in identifying and achieving health objectives. Methods: A pragmatic, unblinded, pragmatic randomized controlled trial involving a delayed intervention was conducted in 32 primary care practices. Results: There was a lower mean rate of hospital admissions in the intervention group compared to the control group (mean ± SD of 0.09 ± 0.33 vs. 0.23 ± 0.60). Conclusions: EPFC did not improve goal attainment and many patient-reported outcomes or experiences.
