Laparoscopic Treatment of Endometriosis and Predictors of Major Complications

Abstract
Background: Laparoscopic surgery is widely recognized in the treatment of endometriosis. Nevertheless, patients could be at increased risk of major complications after surgery.
Purpose: The purpose of this research was to determine predictors of major complications after laparoscopic endometriosis management.
Methods: This retrospective research was conducted at (Omomah center for endoscopic surgery and El Shefaa Hospital) in the period between July 2018 and July 2020 and encompassed 80 patients who had undergone laparoscopic treatment of endometriosis. Characteristics for all women who had laparoscopic endometriosis treatment were demonstrated by the existence or lack of major complications.
Results: There was statistically significant difference between patients without complications and those without complications regarding infertility (p=0.038) and GnRH agonist treatment (p=0.016). There was statistically significant difference between patients without complications and those without complications regarding lysis of adhesion (p=0.042), ureterolysis (p=0.019), total number of procedures (p=0.002), EBL (p=0.002), operative time (p=0.001), and same-day discharge (p<0.001). Multivariate analysis revealed that significant predictors of major complications were lysis of adhesion (p-0.011), ureterolysis(p-0.006), and total number of procedures (p-0.018).
Conclusion: Ureterolysis, adhesiolysis, and an increased number of procedures could indicate perioperative complications. This suggests that surgical complexities, compared to disease severity, could raise the possibility of complication
