Management of Pelvic Organ Prolapse

Abstract
Pelvic organ prolapse is defined as the descent of the vagina and/or uterus below their normal anatomical position this includes the anterior vaginal wall, posterior vaginal wall, the uterus or the vaginal vault after hysterectomy. NICE guidance considers the PFMT as first line in in treating the symptoms of POP. It includes a detailed program of exercises designed to improve the strength and function of the pelvic floor. The first decision in the treatment pathway lies between a woman undergoing obliterative or reconstructive surgery. Obliterative surgery or colpocleisis is undertaken in approximately 5% of women in the USA and involves vaginal procedure that closes the vagina returning the extruded material into the pelvis. The surgery is usually undertaken in the elderly or medially compromised who are happy to sacrifice vaginal penetration and is an effective procedure with quick recovery and low morbidity. The majority of women undergoing surgery for POP will undergo reconstructive surgery that aims to repair defects of the anterior, posterior, or apical compartment
