Abstract
Chronic diseases, such as diabetes, hypertension, and heart disease, have become increasingly prevalent in modern society. These conditions often require long-term management and can have a significant impact on a patient's quality of life. Ensuring that patients with chronic diseases adhere to their prescribed treatment regimens is crucial for maintaining good health outcomes and improving overall well-being. In this context, the role of pharmacists has become increasingly important in supporting patient compliance and enhancing the quality of life for individuals living with chronic diseases.
Pharmacists, as integral members of the healthcare team, are uniquely positioned to play a vital role in the management of chronic diseases. With their specialized knowledge of medication therapies, pharmacists can work closely with patients, physicians, and other healthcare providers to optimize treatment plans, monitor medication adherence, and provide comprehensive patient education and support.
